Dear Ms. Beth Morrison,

 


This letter is to inform you that I am the traditional and legal parent of Jocelyn Nevaeh-Dennea Jackson, born on October, 28th. 2015. My parental rights have not been terminated.

  


As my child’s legal parent, I wish to continue making all decisions pertaining to the rearing and upbringing of my child. I have not, nor do I intend to forfeit any legal right afforded to me as a parent.

  


I wish to be consulted and given an opportunity to consent for all medical care my child should need with our physician. My child’s physician is Dr. Gainer or PA-C Crystal Rucker at Newburg Clinic,  P.O. Box 159 Newburg, WV. 26410, 304.892.2812. All my child’s medical needs should be handled through her.
